§ 210-040. APPLICATION

Application for the permit herein required shall be by an application under oath in writing on a form to be provided by the Sheriff of Sutter County, which shall state the following information material:

(a)

Name and description of the applicant.

(b)

Permanent home and local address of applicant.

(c)

Brief description of the nature of the business and the goods to be sold, or the purpose of solicitation of funds.

(d)

If applicant is the employee of another, the name and address of said employer.

(e)

One photo identification, such as a driver's license photo.

(f)

A statement of the arrest record of applicant, if any.
